Position Summary:



Acts as the primary client advocate, keeps abreast of progress, and coordinates all services as delivered to the mothers and family internally and through external referral.



Essential Job Functions:




+ Compiles Coordinated Assessment reports and SOAR reporting with coordinators monthly.

+ Functions as the single point of contact for Care Coordination with managing entity.

+ Completes reports as required.

+ In coordination with Project Director oversees the day-to-day performance of employees as needed.

+ Managing employee schedules as needed.

+ Often responsible for reporting team and individual performance to human resources and senior management.
