Position Overview
Scope of Services -Tuia Ngā Taura Kaiāwhina
The service supports whānau whose tamariki have been discharged from Waikato Hospital, or who have been referred from a Waikato outpatient clinic or Kāinga Ora and includes the following:
- Working alongside whānau to complete a whānau hauora assessment (WHA), conducted either during home visits or via phone to whānau.
- Providing health and social service education, with referral to additional services as required.
- Supporting whānau to health and social services as required including advocating for whānau at appointments if appropriate.
- Participation in multidisciplinary team hui and collaborative care planning.
- Working as part of a nurse–kaiāwhina team, escalating any clinical concerns to the nurse or GP.
- Speaking and presenting to external services as part of the team to promote the benefits of the Tuia Ngā Taura service and to encourage appropriate referrals.
