Overview
Our client is seeking a Full-Time Weekend Start of Care (SOC) Nurse (RN) to promote the delivery of coordinated, comprehensive care to patients through assessment, planning documentation, and provision of direct nursing care. The ideal candidate will have an Associate Degree in Nursing, at least two years of RN experience, and be CPR certified. Responsibilities include accurately completing assessments, administering skilled nursing care, and collaborating with other healthcare professionals to ensure patient care needs are met.
Key Responsibilities
• Accurately completes required start of care documentation on new admissions, post-hospitalization.
• Assesses the patient for home telemonitoring, incorporates telehealth into plan of care, and ensures notification of patient admission to the telehealth program to the telehealth department in established timeframes.
