Job Summary:
The Ambulatory Transplant Pharmacist is a designated member of the transplant team and is assigned primary responsibility for providing comprehensive pharmaceutical care to organ transplant (heart, liver, lung, kidney, and pancreas) recipients. The pharmacist evaluates, identifies and solves medication related problems for transplant recipients and is a liaison between patients and patients' families and other health care team members regarding medication issues. They work with patients and their families and members of the transplant team including physicians, nurses, and coordinators to contribute to the continuity of care for each transplant patient. They also collaborate with physicians, nurses and other healthcare professionals to ensure the delivery of safe, effective, and cost-efficient medication treatment.
