We are looking for a Case Manager with strong skills to join our Portland team with the ability to manage a busy schedule, the needs of each family, and documentation needs. This is a hybrid position where the Case Manager will be located at Project Home Sites supporting transition-aged youth accessing Foster Youth to Independence Vouchers (FYI) through Project Home's FYI Program.

DUTIES:

Case Managers are the central piece to a network of providers working with each child. You will work to help coordinate and collaborate with each provider working with the child to ensure that their needs are being addressed - home needs, school needs, medical needs, and/or community needs.
