Travel PT Home Health jobs in Northeast Philadelphia, PA are available for immediate start with 13-week assignments. You will work 8-hour days, completing 5 to 6 visits per day, and use Homecare Homebase EMR. Required qualifications include home health experience and a Pennsylvania license.Your responsibilities include evaluating and treating adult and geriatric clients in their homes, developing individualized care plans, documenting progress, and educating clients and families on rehabilitation and safety.
