Traveler Cover Page
Banner has a specific cover page! PLEASE vet all locations to the travelers and add all experienced departments candidates are willing to ACCEPT/work in. This helps us get candidates presented quicker as we all know these orders open/close quick! This form needs to be 100% accurate and have all requests noted in the highlights section.
- Start Date should be on or very quickly after the Order Start Date listed.
- All RTO should be listed on the cover page, must be less than 7 days.
- RTO will not be approved in the first week of assignment or holidays.
All facilities the traveler is open to working in should be listed on the submission. - If interested in any location, please include what states (I.E. Any AZ, Any CO, Any state, etc.)
