Position Overview:
The Substance Use Disorder (SUD) Case Manager is responsible for the overall coordination of care and supporting clients/clients through their recovery journey. The SUD Case Manager collaborates with clients, the treatment team, and external providers to coordinate discharge and aftercare plans, ensuring a smooth transition to the next level of care. They regularly meet with clients to assess needs, identify suitable referrals, and secure aftercare appointments. The Substance Use Disorder (SUD) Case Manager will also be cross-trained and scheduled for Behavioral Health Technician (BHT) shifts. Key Responsibilities:
