Squirrel Hill Health Center is hiring a Substance Use Disorder (SUD) Program Coordinator to support recovery, improve health outcomes, and sustain public health initiatives like harm reduction and medication-assisted treatment (MAT). ​ This role serves as a liaison between patients, providers, staff, and community partners, while coordinating all aspects of the SUD program. ​

Responsibilities: ​

+ Coordinate the SUD/MAT program and track patient progress. ​

+ Conduct patient assessments, create care plans, and provide education on harm reduction and overdose prevention. ​

+ Facilitate referrals to behavioral health, counseling, and social services. ​

+ Maintains program standards of COE, MAT and SUD programming, including workflows and documentation for providers and patients.

+ Collects, tracks, and reports program data in REDCap and other reporting systems. Prepares reports for MCOs, PittPERU, funders, and accrediting agencies.​
