Key Responsibilities:
• Participate in Comprehensive Geriatric Assessments as a team member
• Complete triage for new patient referrals
• Collect and analyze clinical data thoroughly
• Deliver consultation services to enhance daily living
• Establish collaborative care plans with the inter-professional team
