**Description:**
This individual is responsible to plan and implement habilitative/rehabilitative programs for individuals who exhibit speech/language/swallowing problems.
**ESSENTIAL FUNCTIONS:**

+ Plans, directs, evaluates, and conducts habilitative and rehabilitative programs to treat speech, language and/or swallowing disorders.
+ Prepares written reports of diagnostic findings, intervention techniques used, progress made and follow-up status in a timeframe according to policy and procedure.
+ Develops treatment plans, directs, supervises, and treats patients/caregiver in accordance with treatment plan.
+ Consults and communicates with patient, family, providers, and nursing staff.
+ Assist multidisciplinary team with discharge planning and establishes home therapy programs.
