ESSENTIAL JOB DUTIES
Reviews social history, psychological, medical, and previous therapy reports/records.Interviews individual/caregiver to discuss and establish needs. Administers appropriate tests and evaluates to determine functional level and potential. Completes all evaluations, documentation, and billing within timelines according to procedure. Communicates information to team participants and provides input for treatment plan development. Makes referral or recommendation for therapy and/or equipment. Develops treatment plan from evaluation results and team input. Conducts individual and/or group sessions within clinic to work on specific objectives.
