M-F, 8hrs with weekend rotation (every 3rd/4th weekend).

Requirements:

• MD LMSW or higher


• 1+ years of discharge planning experience in an inpatient setting


• Must have EPIC experience

Responsibilities: Early Identification, high-risk screening, and readmission screening of all patients. Services include: adjustments to illness counseling, safety assessments, discharge planning, identification of obstacles and barriers to discharge through proactive screening and assessments, and coordination with the multi-disciplinary team.
