Responsibilities:
Conduct urgent home visits within the community.Complete Care Act Assessments and develop Support Plans.Update detailed and accurate case notes.Undertake Mental Capacity Assessments and make best-interest decisions.Collaborate with the Home First Team multidisciplinary team (MDT).Carry out safeguarding initial enquiries.
