What will your responsibilities be?
In this role, you will:
Conducting Assessments and Care Planning: Carry out strength-based assessments of adults in the community to determine care needs, risks, and eligibility for support under the Care Act 2014.Safeguarding Vulnerable Adults: Respond to safeguarding concerns, investigate allegations of abuse or neglect, and coordinate protection plans in line with statutory guidance.Multi-Agency Collaboration: Work closely with health professionals, housing teams, voluntary organisations, and other stakeholders to deliver integrated support and improve outcomes.Promoting Independence and Wellbe...
