Summary The Social Worker provides comprehensive psychosocial services to Veterans receiving care through the Community Living Center (CLC) and Palliative Care Program. The incumbent conducts assessments, develops treatment plans, provides counseling and case management services, coordinates care with interdisciplinary teams, and connects Veterans and caregivers with appropriate VA and community resources.
