**Job Summary and Responsibilities**

As our Social Worker you will proactively assess, develop, and implement comprehensive plans of care in collaboration with patients, families, and an interdisciplinary team.

**Essential Functions**

+ Completes psychosocial assessments, interviews patients, relatives, friends or significant others to obtain information about the patient’s personal, social and emotional history.
+ Provides therapeutic care to patients to assist them in accomplishing treatment goals using interventions and techniques that are appropriate to the age and developmental level of the patient.
+ Coordinates treatment with community agencies such as social services, schools and probation.
+ Maintains social service records of each patient according to hospital and departmental program standards. Maintains statistics and turns in reports in a timely manner.
