Provide timely and comprehensive social work biopsychosocial assessment, consultation, treatment and discharge reports, including risk, crisis, and clinical needs assessments.
Provide individual, family and group therapy following best practice standards, consistent with the Ontario College of Social Workers and Social Service Workers.
Complete risk assessment and crisis intervention plans.
Engage client, family and caregivers in the clinical interventions and risk reduction strategies with the interdisciplinary team.
Advocate on behalf of clients, families and caregivers for appropriate services and resources.
Collaborate, consult and liaise with team members and community resources.
Demonstrate thorough knowledge of the theories and practices of a professional social worker.
Maintain workload measurement and data collection as required (. RAI).
