Main Responsibilities Developing and implementing
individualised care plans
for residents.
Monitoring residents'
progress and well-being
and adjusting care plans accordingly.
Planning and conducting
programmes and activities
to meet residents' care needs.
Preparing
monthly case notes and 6-monthly review reports
for assigned residents.
Ensuring adherence to
SOPs and care standards
in service delivery.
Requirements Degree
or
Graduate Diploma in Social Work, Psychology, or Counselling.
1+ year
of relevant experience in
social service / care settings.
Willing to perform
shift work based on duty roster.
