Assists patients in understanding their treatment options and the resources available to them within the Baptist system and the surrounding community as well as helping to resolve barriers to accessing primary psychiatric and preventive healthcare service in light of anticipated discharge.Establishes and maintains positive working relationships within the behavioral healthcare community, as well as those business in the community at large, that can ensure patients receive the care and services needed.Educate patients in regards to where and when and how to access behavioral healthcare services including the roles played by the various outpatient behavioral health resources post discharge to aid in the obtainment of optimal recovery outcomes.Assists in discharge planning and provides appropriate li...
