Short Term Services Support Worker

Our short‑term services are recruiting for a Support Worker to join our team on a part‑time (30hrs) permanent contract. You will provide care and support to people in the community with a focus on maximising independence and delivering a high standard of care to a variety of people, including end‑of‑life care.

Following a period of acute ill health or hospitalisation, you will undertake short‑term care as defined in care & short‑term service plans or delegated tasks from Therapists/Nurses, contributing to the assessment process and ongoing monitoring and review of people’s goals and reporting any changes to a person’s medical condition.

This role is part of an exciting development of services to help prevent avoidable hospital admissions, working alongside intermediate care teams and other community teams.
