Key accountabilities
Review medication orders for completeness and appropriateness, and if needed, perform and document interventionsProcess drug orders and dispense medications to patientsCounsel patients (inpatients / outpatients) on their medications and provide specialised counselling if neededPerform medication reconciliationProvide drug information to doctors, nurses and patientsAssist in the preparation of ad-hoc items e.g. extemporaneous preparations, reconstituted antibiotic suspensionsParticipate in continuing professional education (CPE) programmesOther duties as assigned Qualifications & Experience
Bachelor of Science in PharmacyRegistered with Singapore Pharmacy Council
