This post is offered as a fixed term contract until 31 March 2027, or as a secondment for internal staff. 

The Senior Case Worker is part of the Hammersmith and Fulham Navigator Service, made up of three Senior Case Workers, a Co-ordinator and a Service Manager.

The Navigator Service provide intensive support to vulnerable adults who are:

• At risk of homelessness, e.g. due to eviction or difficulties sustaining their tenancy,
• In Supported Housing and need intensive support to be able to move on into their own independent tenancy
• Homeless (e.g. sleeping on the streets / sofa surfing / leaving hospital or prison with no accommodation) and are needing intensive support to find suitable accommodation
