LOCATION: Remote – must live in North Carolina or within 40 miles of the North Carolina border. Incumbent must be in/near Region 3 counties which are: Alexander, Anson, Cabarrus, Catawba, Cleveland, Gaston, Iredell, Lincoln, Mecklenburg, Rowan, Stanly, and Union. Travel is expected and required for this role.

GENERAL STATEMENT OF JOB

The Rural Health Transformation Program (RHTP) is statewide program administered by the NC Department of Health and Human Services (DHHS). The purpose of the RHTP is to create sustainable improved access and outcomes to NC citizens living in rural communities. This program is funded by the US Centers for Medicare & Medicaid Services (CMS).
