**_PATIENT NAVIGATOR_**

**Location:** Baylor All Saints – Fort Worth

**Specialty:** Transplant

**Shift:** FT Days, Monday-Friday, 8:00am-4:30pm; flexibility is needed for community events after work hours. This position will also require TRAVEL to West Texas and Central Texas on a quarterly basis.

**Experience:** 3 years of recent RN experience

**Preferred but not required:** Spanish speaking, Transplant experience, Community Health experience, Practice Liaison experience
