Position Overview
Provides skilled nursing care focusing on pain control and symptom management. Responds to medical or emotional crisis and/or death as requested by designated triage nurse. Conducts hospice admissions as needed. Consults appropriately with attending physician and medical directors to coordinate patient care and receive orders. Assists RNCMs with their patient assignments by conducting scheduled after-hours visits as necessary to meet patient care needs. Educates patients/caregivers on hospice philosophy, disease progression, physical components of care, stages of dying, and other aspects of the POC as necessary.
Required:
1. Associates Degree in Nursing
2. NJ Nursing License
3. Unrestricted NJ driver's license with clean record
Preferred:
1. Bachelor's degree in Hospice, Palliative Care, or related field.
2. 2 years of experience in Hospice, Palliative Care, or relevant area.
