Duties & Responsibilities:

Performs Patient assessments: Accurately assesses patient’s appropriateness for hospice care, learning needs and progress toward goals. Develops the plan of care for the patient and family

Ensures appropriate utilization of services: Maintains visit productivity standards. Demonstrates knowledge of insurance reimbursement

Completes discharge assessments: Performs laptop transfer of data twice daily. Completes admission assessment and transfers within 24 hours. Completes re-visit documentation and transfers

Uses age appropriate teaching methods for patients and families: Evaluates teaching plan and revises as necessary. Documents patient education tools used

Assumes responsibility for Case Management: Develops/implements/evaluates plan of care based on evidence-based knowledge. Revises care plan as indicated. Prioritizes care needs. Demonstrates knowledge of hospice regulations and insurance reimburs...
