This is a fully remote role where you’ll play a key part in helping patients navigate complex, chronic conditions while contributing to a rapidly scaling care model.

What You’ll Do
• Build ongoing relationships with patients through monthly virtual check-ins to track progress and adjust care plans
• Support patients with neurological (Alzheimer’s, Parkinson’s, MS) and endocrine conditions (diabetes, thyroid disorders, metabolic conditions)
