Position Overview
IKC RN Case Manager
Arrival: Typically starts from home, checking emails/dashboard for hospitalized or high-risk patients.
Primary Tasks:
- Caseload Management: Reviewing a roster of high-risk patients (often ~100+).
- Field Visits: Traveling to dialysis centers or nephrology offices to meet patients during their treatment/appointment.
- Assessments: Conducting Comprehensive Health Evaluations (CHEs)βhead-to-toe assessments covering medical, social, and emotional needs.
- Education & Coordination: Teaching patients about comorbidities (diabetes, heart failure), reconciling medications, and arranging follow-up care to prevent hospital readmissions.
- Documentation: Heavy focus on documentation for risk adjustment and care plans (often completed from home).
Outcome Focus: Reducing hospitalizations and managing overall health long-term.
