Role and Responsibilities


Administer direct skil l ed nursing care to patients , in accordance with physician’s orders .





Essential Functions


+ Provide hands-on , skilled nursing care in accordance with physician’s orders .

+ Provide health education to the patient and family .

+ Assess patient response to the plan of care interventions and communicate changes to the case manager .

+ Collaborate and share findings with the multidisciplinary care team .

+ Document those services rendered to the patient, note changes in the patient’s condition and/or family and home situations.

+ Document communication with case manager and physician.
