Position Summary



Provides expertise in assessment, plan development, coordination, and monitoring of care services to ensure the highest quality of care . Acts as a guide for our Clients by navigating the complexities of the health care system.

Assists in making decisions from a neutral point of view as family members sometimes have to make difficult decisions that are deeply personal.

CCM's enhance the quality of life in our Clients by organizing care plans and advocating Client's needs by working closely with social workers, attorneys, healthcare providers, and other providers in collaborative efforts to support some of the frailest of our community.




Essential Duties and Responsibilities

• Evaluates in-home care needs for concierge and premium clients


• Addresses emotional needs and concerns
