Summary The Ralph H Johnson VA Medical Center is seeking a Cancer Care Nurse Navigator to coordinate the cancer patient's journey through the health care system across disciplines and sites. This position is essential to the Cancer Program at the facility, and all activities of the Nurse Navigator are dedicated to the patients with cancer and the cancer care team. Responsibilities Duties include, but are not limited to: Executes position responsibilities that demonstrate expert leadership, experience, and creative approaches in providing complex patient management. Provides patient management, care coordination, and disposition planning for cancer patients. The Cancer Care Navigator assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs.
