Clinical Coordinator / Rehab Case Manager - Austin, TX - Rehab Without Walls
Who we're looking for:
+ Seeking an experiencedlicensed clinician(PT/OT/SLP/LCSW) tocoordinate careand support a consistent rehabilitation experience for patients with neurological diagnoses.

+ Serves as theteam leadand is a central point of contact patients, families, providers, and the interdisciplinary rehab team.

+ Supports patient recovery in home and community settings to promote independence and reintegration into daily life.

+ Ideal for professionals with 3+ years in rehabilitation settings, and/or case management experience similar to inpatient rehab settings
