The RN Transfer Center Specialist coordinates patient transfer logistics using workflows identified by client to assist with patient acceptance and bed placement. This RN understands cases are related to ensuring patient care is accepted in an environment that can facilitate a higher level or care or capability as well as continuity of care. Performs functions based on defined standards of performance and practice to meet or exceed operational expectations. The position works collaboratively to foster relationships with referring facilities, physicians, and hospital staffs in representation of Conduit Health Partners Mission and Values.
