Team 5: Bohemia, Brentwood, Fire Island, Great River, Islandia, Islip , Islip Terrace, Lake Ronkonkoma, Oakdale, & West Sayville.

+ 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends

+ Per Visits are required to work at least 1 shift per week

+ Per visits are required to attend entire first week of orientation Monday – Friday (8:30AM -4:30PM.)


Job Details

Our Home Care RN’s:

+ Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.

+ Measure vital signs, Administer IVs, change dressings, clean wounds as needed
