**Registered Nurse (RN) – Field Case Manager**

**Location:** Field-Based / Travel (Must reside in Illinois; primary coverage area includes Wayne County and the greater Macomb/McDonough County regions)

**Compensation:** $46.66 per hour + Mileage/Travel Reimbursement

**Licensure:** Active, unrestricted Illinois RN License required

**Role Overview**

As a Field Case Manager, you will play a pivotal role in advocating for members right in their communities. Moving beyond a desk, you will facilitate a collaborative process of assessment, planning, and advocacy through direct, face-to-face interaction. You will utilize clinical tools, localized data, and regional healthcare networks to evaluate member needs, optimize care plans, and ensure cost-effective, high-quality outcomes. This is an active, fast-paced role that relies heavily on clinical expertise, independent decision-making, and regional travel.
