**Description:**

**POSITION PURPOSE**

+ The Registered Nurse Case Manager assumes overall accountability for patients’ discharge plans while assuming a leadership role in achieving desired clinical, financial and resource utilization outcomes.
+ This is achieved by coordinating care and service for defined patient populations during an acute care episode across the continuum of care.
+ This position is a Part-Time, benefited position, scheduled 40 hours per bi-weekly pay period: Orientation for this position would last for 6-8 weeks from 8 a.m. to 5 p.m. Monday through Friday.

**WHAT YOU WILL DO**

+ The RN Registered Nurse Case manager is responsible for conducting a focused assessment of patient goals, as well as, biophysical, psychosocial, environmental, economic/financial and discharge planning needs.
