Description

Registered Nurse, Assertive Community Treatment (ACT) - PER DIEM

Per Diem Schedule: Monday - Friday during normal business hours with a minimum commitment of 8 hours per week

As an ACT Registered Nurse, you will assess patients with mental disorders and provide medical care and medication management for patients in your care. You will be responsible for ensuring that patients have access to basic preventative care and treatment of mental health and medical needs. You will report to the Director of ACT, Licensed Professional, and work onsite and in the community.

Responsibilities:


+ Maintain clinical records on all patients seen in a manner consistent with confidentiality of health care records guidelines and department regulations protocol.

+ Support treatment planning and medication needs of all clients.
