Summary The VA New Mexico Healthcare System is seeking a Registered Nurse to serve on the Patient Aligned Care Team (PACT) - Outpatient Staff. They are responsible and accountable for all elements of the nursing process, including assessment, planning, implementation, and evaluation of age-specific care. Coordinates patient education and self-management, administers medications and procedures, collaborates with interdisciplinary teams, and demonstrates leadership in managing complex patient care.
