Summary The Registered Nurse - Outpatient Patient Aligned Care Team demonstrates integration of biopsychosocial concepts, cognitive skills, and technically competent practice while providing care to clients. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Responsibilities The Patient Aligned Care Team (PACT) Registered Nurse (RN) is responsible for providing competent, evidence-based gender-specific primary care to patients with oversight of licensed vocational nurses/nursing assistants as appropriate to the clinic setting. This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.
