10:45pm to 7:15am Serve as a Charge Nurse, Staff Nurse, or Assessment Coordinator within a long-term care setting dedicated to Veterans’ healthcare services.Provide direct nursing care to Veteran residents with chronic medical conditions, physical disabilities, cognitive impairments, behavioral health needs, and age-related health concerns.Conduct comprehensive nursing assessments, including full physical assessments, cognitive and mental status evaluations, and ongoing condition monitoring.Develop, implement, and evaluate individualized nursing care plans in collaboration with interdisciplinary care teams focused on Veteran-centered care.Coordinate care with physicians, specialists, therapists, social workers, Veterans, and family members or designated support per...
