Position Overview
Duties & Responsibilities:
- Performs patient assessments: Accurately assesses patient’s appropriateness for hospice care, learning needs, and progress toward goals. Develops the plan of care for the patient and family.
- Ensures appropriate utilization of services: Maintains visit productivity standards. Demonstrates knowledge of insurance reimbursement.
- Completes discharge assessments: Performs laptop transfer of data twice daily. Completes admission assessment and transfers within 24 hours. Completes re-visit documentation and transfers.
- Uses age-appropriate teaching methods for patients and families: Evaluates teaching plan and revises as necessary. Documents patient education tools used.
- Assumes responsibility for Case Management: Develops, implements, and evaluates plan of care based on evidence-based knowledge. Revises care plan as indicated. Prioritizes care needs. Demonstrates knowledge of hospice regulati...
