Summary The VA Western New York Health Care System is seeking an intermittent Registered Nurse (RN) to serve as a member of the Home Based Primary Care (HBPC)Team and provide care to Veterans living in the Batavia, New York area. Commute to additional surrounding areas may be required on rare occasions. The HBPC RN plans and delivers technically competent care in the home of Veterans with complex needs, including health promotion, health prevention, geriatric care, primary care, and palliative care. Responsibilities The Home Based Primary Care (HBPC) RN provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the HBPC Program. The RN is responsible for providing competent, evidence-based care to HBPC patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting.
