Comunilife is looking for an experienced Registered Nurse Assessor for its Adults Home program in Queens. The RN functions as the medical/physical health lead on the Housing Provider Team responsible for assessing class members’ service needs currently and upon transition to their supported apartments. The RN assesses plans and evaluates the physical health and medical needs of class members with SMI before transitioning to community and scattered-site housing.
