The Palliative Care RN will provide care coordination, assessments, education, and transitional care management. The RN will work amongst an interdisciplinary supportive care team with consultations from pain, palliative care, hospice, bereavement, and complementary medicine backgrounds. The team will coordinate patient care needs for outpatient, inpatient, and follow-up care including: symptom management, pain management, quality of life assessments, psychosocial support, effective coping strategies, support resources, aromatherapy, music therapy, massage therapy, hypnosis, review of medications, and end of life decision making. The RN will be expected to participate in inpatient rounds, outpatient clinic, and complete some bereavement follow-up care. The RN will establish a consistent, standardized best practice across inpatient and ambulatory settings for these patients and families while improving satisfaction and quality of care for these children....
