**Overview**
**REACH Kidney Care** , a division of Dialysis Clinic, Inc., is a kidney health management program designed to benefit patients along the continuum of kidney disease through health education, self-management, behavior change counseling, coordination of care with other providers, and patient navigation services. This program includes assisting patients with advanced chronic kidney disease (CKD) and end-stage renal disease (ESRD). The goal of the REACH care team is to educate, support, and empower self-care to improve kidney health and delay the need for renal replacement therapies (RRT). For people who do progress to ESRD, the REACH care team aims to increase preemptive renal transplants (before the need for RRT), facilitate a successful transition to RRT or medical management (MM), increase renal transplant rates after the start of dialysis, and address advance care planning.
