Job Description/Requirements.

The Licensed Clinical Social Worker serves patients of the Network. He/she is a member of the patient-centered health care team at the assigned location(s), and coordinates care activities with other Providers.

**Essential Duties and Responsibilities:**

+ May be assigned to offer “traditional” mental health services, which includes assessment, diagnosis, establishment of treatment goals and care plans, provision of mental health care and coordination
+ Attentive to general medical concerns and treatments, and supports such interventions
+ May be assigned to an “integrative” role, in which case he/she serves as a Behavioral Health Consultant (BHC) to a defined set of primary care providers
