Based: Various locations in Northampton. We are looking for passionate and accomplished Social Workers to join our Learning Disability Team in West Northamptonshire.

Teams we work with

• Community Hub teams (across Northamptonshire, including rural areas such as Daventry, Towcester and Brackley) – support a wide range of adults 18+ with a variety of needs.
• Learning Disability team – support adults specifically with learning disabilities across West Northamptonshire.
• Flow and Capacity team (FACT) – undertake assessments, advise and coordinate support for people in hospital or discharged home for reablement, and manage transfers to settings such as residential discharge to assess placements or the Recovery and Independence Bed Unit.
• Assurance and Safeguarding team – minimise risks to people, work with those aged over 18 for whom there are concerns about abuse or neglect, and carry out investigations surrounding these concerns.
