The Psychiatric Mental Health Nurse Practitioner will work with patients in providing outpatient community mental health services. The Psychiatric Mental Health Nurse Practitioner will collaborate with the Primary Care Providers to recognize, treat and manage mental health and psychosocial issues while acting as a contributing member to the Integrated Primary Care Team.
RESPONSIBILITIES
1. Provides focused intervention for patients who are in need of mental health services.
2. Provide counseling, rehabilitation and support services.
3. Talk, listen, observe and analyze patients and suggest a suitable mode of treatment. They may refer to specialists for treatment.
4. Suggest therapy in groups or individually as a mode of treatment.
5. Provides feedback to the Primary Care Provider via documentation in Medical Record.
6. Initiates follow up to determine any changes in treatment approach.
