The Community Health Center, Inc. cares for 150,000 patients every year at sites across the state. We know that many of CHCI patients have complex lives and taking care of their own health can’t always be the number one priority. CHCI is excited to have a Preventative Cancer Screening Navigator join the team to help our patients prioritize these potentially life-saving screenings. The Navigator will work closely with the Population Health team to identify patients who are due for colorectal and/or breast cancer screening. This person will play an important role in the care team by supporting the patient in overcoming any barriers that are preventing them from completing the recommended screenings such as a mammogram or colonoscopy.
